Primary Care Nursing Services
Patients must be referred as appropriate by a GP, a hospital consultant or doctor, charge nurse or an Allied health care professional.
The district nurses manage the care of house bound patients within the community. Their duties generally include the following:
- General Wound management including leg ulcers.
- Catheter care.
- General assessments.
- The administration of injections including insulin.
- Palliative care.
- Ear syringing.
- The administration of the influenza vaccine, pneumococcal vaccine and the Herpes Zoster Zostavax vaccine.
The district nurses work very closely and collaborate with the community multidisciplinary groups, such as the physiotherapists, occupational health visitors, palliative care nurses and hospice nurses. Their work involves discussing patients’ care plans for a positive health-care outcome to prevent unnecessary or avoidable hospital admissions.
